A Study of Chronic Gastrits in the Rescted Stomach


Abstract
Histopathological studies were made from 155 Korean stomachs which were taken surgically from patients with duodenal ulcer, gastric ulcer, and gastric carcinoma for the conditions, degrees of gastritis and incidence of intestinal metaplasia according to their respective sites.
The main results are as follows:
1. A chronic gastritis was present histologically in all materials studied.
2. In the surgically taken materials the picture of gastritis is different greatly according to its sites, and generally it is more severe in the pyloric portion and lesser curvature, and mild in greater curvature.
3. In the duodenal ulcer cases there were more milder forms of gastrtis and in the cases of gastric ulcer and gastric carcinoma there were more cases of severe gastritis.
4. The incidence of chronic atrophic gastritis are as follows:
a. In duodenal ulcer there were 28.1% in pyloric portion, 23.8% in lesser curvature, and 4.8% in greater curvature.
b. In gastric ulcer there were 51.7% in pyloric portion, 41.7% in lesser. curvature, and 30.0% in greater curvature.
c. In gastric carcinoma there were 68.5% in pyloric portion, 50.7% in lesser curvature, and 41.7% in greater curvature. From the above we find that the incidence of chronic atrophic gastrits is highest in gastric carcinoma, next in gastric ulcer and the least in duodenal ulcer.
5. Generally speaking, the incidence of severe gastritis. particularly chronic gastritis was higher among the older age group than in younger age group.
6. The intestinal metaplasia was not found in the normal tissues, but there were 9.4% in the surperficial gastritis, 44.5% in the borderline form, and 83.8% in the atrophic gastrits. Thus, the incidence and the degree of intestinal metaplasia is higher when there is severe gastritis.
7. In the incidence of intestinal metaplasia accordieg to the diseases there were 42.9% in duodenal ulcer, 68.3 in gastric ulcer and 93.2% in gastric carcinoma.Thus the incidence of intestinal metaplasia is highest in gastric carcinoma, next in gastric ulcer and the least in duodenal ulcer. The degree of the intestinal metaplasia was more of mild cases in the duodenal ulcer, and more of from mild to moderate cases in the gastric ulcer and more of from moderate to advanced cases in the gastric carcinoma.
